Whether you are preparing for baptism, planning a funeral, becoming a member, volunteering for a parish ministry, or simply staying in touch through our parish email list, you’ll find the forms you need below.

Please Note:

  • Some of the forms below are fillable PDFs.          You may complete them online and then print the finished form, or download the blank form, print it, and complete it by hand.  All other forms are fillable on screen.  All personally identifiable information on these forms is securely stored.

Please return any hand-completed form to the parish office (or bring it with you when you meet with the rector). All information will be kept confidential and used only for parish ministry.

Baptismal Information Form

We are delighted that you are preparing for the Sacrament of Holy Baptism. This form gathers the details we need both to prepare for the service and to record the baptism in our official parish register.

Required Marriage Forms

We rejoice with you as you prepare for the sacrament of Holy Matrimony. This form gathers the information we need both to plan your wedding service at St. James and to record your marriage in our parish register.

New Member Information 

We are glad you are considering making St. James your church home. This form helps us get to know you, welcome you into parish life, and record your membership in our official parish register.

Click the button below to complete the form. When you click SUBMIT, your information will be sent directly to our Rector. All personally identifiable information (PII) is stored and transmitted securely.

Funeral Liturgy Planning

This form allows you to record your wishes for a funeral or memorial service at St. James. Having this information in advance is a great gift to your loved ones and helps us prepare with care and reverence.
